Check the FC entry on EA's server status page and @EASPORTSFC for outage updates. If FC is online, run the connection checks, and look at your score in EA's Connection Quality Report.

Yes. EA lists these account problems that block online play: a banned or suspended account, a deactivated or deleted account, an account already in use elsewhere, and an underage child account.

Make sure your PlayStation online subscription is active, and that any EA Play for PlayStation membership has not lapsed. Restarting the PlayStation in Safe Mode clears its cache.
