Open the game, go to Settings, Help and Support, and start a conversation there. In-game support carries your village details automatically, which a web form cannot.

Recover it through the Supercell ID the village was linked to. Without a Supercell ID, recovery is much harder, which is why linking one before you switch devices matters.
